FAQs About Immunization Requirements for School Admission

Please note the meningococcal vaccine requirement for students entering grades 7-12 requires students in these grades to show proof of one dose of quadrivalent meningococcal conjugate vaccine (MCV4 or MenACWY) on or after the student’s 11th birthday.

In addition, the requirement for Hepatitis A vaccine includes students through Grade 12.

Exemptions from Immunizations

For medical reasons

  • The law allows physicians to write a statement stating that the vaccine(s) required would be medically harmful or injurious to the health and well-being of the child. The written statement by the physician must be submitted to the school nurse. Unless it is written in the statement that a lifelong condition exists, the exemption statement is valid for only one year from the date signed by the physician.

    For reasons of conscience, including religious beliefs

  • The law allows parents/guardians to choose an exemption from immunization requirements for reasons of conscience, including a religious belief. The completed and returned form from the Texas DSHS must be submitted to the school nurse in order for the student to be admitted. The school will accept only the original official affidavit forms developed and issued by the Texas DSHS. The affidavit will be valid for two years.
  • Please note: In the case of an outbreak, a child who is not immunized may be prohibited from school attendance.
  • Any form presented for conscientious objection to immunization must be presented before the student may start school and must be updated every two years by the anniversary date.

    Because the student had the disease

  • Parents of students who had the chickenpox disease (varicella) may submit a signed form to the campus nurse that indicates the date that the child had the disease. It may also be documented by the physician on the immunization record. Medication Policy
According to Section 22.052, Education Code, any medication administered to a student in a Texas school must have a written request/authorization from the student’s parent or legal guardian and must be in the original container and be properly labeled.

Exclusion Policy 
For the protection of all students the following policies are in place and will be strictly enforced. A student will be sent home with: (list not all-inclusive)

  • Fever 100.0° or higher
  • Suspected contagious disease
  • Vomiting or Diarrhea
  • Rash on any part of the body
  • Red eye with watering and/or crusting
  • Feel too bad to remain at school

If your child has any of the above symptoms in the morning before arriving at school, Please Keep Him/Her At Home. A child with a cold or cough should be kept at home for observation. We have limited facilities for caring for ill children at school and ask that you make arrangements for the child to be picked up as soon as possible when contacted by school staff. Please enter the building and sign the student out in the office. Your child should remain at home until he/she remains free of fever for 24 hours without the use of medication.
